Form Instructions

ONE: When you access the form, make a copy of the file.

TWO: Complete the fields required – there are comments on most fields indicated by a black triangle in the upper right corner of the cell; hover over or click in the cell to display the comment. Do not make adjustments to any cells shaded in green.

THREE: Email the completed form and related receipts to the applicable approver.

FOUR: The approver should forward their approval with the completed form and receipts to [email protected] and [email protected].
